Diagnosis and Management of Sciatic Endometriosis at the Greater sciatic Foramen: a Case Report

Abstract Sciatic endometriosis is a rare condition in which endometrial tissue invades or compresses the lumbosacral plexus and/or sciatic nerve and causes pain, often cyclical. Its diagnosis depends on recognition of signs symptoms atypical to common degenerative lumbar disorders its treatment requires timely coordinated care. A 26-year-old woman presented chiropractor at hospital-based outpatient clinic with 6-month exacerbation radiating pain paresthesia from right gluteal region into leg foot. She was previously treated for over 3 months suspected radiculopathy physical therapy. Multisegmental neurologic deficits inconsistent her prior imaging prompted further investigation. Pelvis hip MRI identified 7×5 cm mass extending through greater foramen compressed infiltrated plexus, nerve, superior confirmed via biopsy be endometriosis. Referral co-management neurologist obstetrician resulted conservative strategy hormone therapy option having surgical excision if needed. disorder that may misdiagnosed due tendency mimic disorders. This case highlights role coordinating care complex benefits early optimal management. It also illustrates how integration chiropractors hospital system can facilitate their ability manage neuro-musculoskeletal as they easily refer communicate other medical specialties within network.
